Tips for Progress Reports!

When preparing to complete progress notes, it is best practice to review the following in your sessions leading up to their due date:
Re-assess clients' progress on each goal. This should be assessed ongoing throughout each 3 month period.
Discuss with parent the child's progress on each goal.
Ask parent/caregiver if they have any new goals or concerns we can address, in collaboration with OTR. New goals can be added during progress note time and thus, a new plan of care developed. Reminder: New goals require completion of the "ISP Change Form" (see attached) and submitted to Tyler (who submits to DDD).
Review home program and adjust as needed, in collaboration with OTR.
Once you have completed your notes, always remember to inform your supervising therapist they are ready for review. Keep up the good work, TEAM CAPABLE!
